Your Email (required for registration)

Telephone Number (required for registration)

Birthdate (required for registration)

Karatbars.com Sponsor ID (Leave blank if you do not have one, or if it is Goldichiba)

1st preference for your own Karatbars.com ID (keep it simple at least 7 characters long)

2nd preference for your own Karatbars.com ID

3rd preference for your own Karatbars.com ID

4th preference for your own Karatbars.com ID (if all 4 are already taken we will create one for you based on it)

Upload Utility Bill, Electric, Gas, Water, Phone etc (Picture must show all 4 corners, required for registration)

Upload Picture ID, Passport, Hokensho, Drivers License etc(Picture must show all 4 corners, required for registration)

Full Address, where orders will be sent (required for registration)
